What Is an FAA Air Traffic Control Medical Exam?

The FAA Air Traffic Control Specialist (ATCS) Medical Exam is a comprehensive physical and mental health evaluation required by the Federal Aviation Administration. It ensures that individuals applying for or maintaining a position as an air traffic controller meet the medical standards necessary to safely perform their duties.

This medical exam is similar to the FAA Second Class Medical Exam for pilots, but with a few key differences that focus on the unique demands of ATC work.

Who Needs an ATC Medical Exam?

FAA medical exams are required for:

  • Individuals applying for federal air traffic controller jobs (especially through the FAA Academy)
  • Current FAA-employed air traffic controllers renewing their medical clearance
  • Certain contract tower personnel or private-sector controllers, depending on the employer

If you're training or working in this role, the FAA requires you to pass this exam annually.

What Does the FAA ATC Medical Exam Include?

Your exam will evaluate:

  • Vision
    • Distant and near vision must be correctable to 20/20
    • Color vision must be normal (Ishihara test or equivalent)
  • Hearing
    • Must be able to hear conversational voice in each ear at 6 feet (with or without hearing aids)
  • Cardiovascular Health
    • Blood pressure, heart rate, and overall heart health will be reviewed
    • An EKG may be required depending on age or medical history
  • Neurological & Mental Health
    • The FAA requires disclosure and review of any history of seizures, neurological disorders, depression, anxiety, or other psychiatric conditions
  • Substance Use Screening
    • Must report any history of alcohol or drug abuse
    • A urinalysis or other tests may be requested
  • Medical History & Medications
    • All current and past conditions, surgeries, and prescriptions must be disclosed

Can You Be Disqualified from ATC Medical Certification?

Yes. Disqualifying conditions include:

  • Uncontrolled diabetes
  • Epilepsy
  • Certain psychiatric conditions
  • Substance abuse disorders
  • Vision or hearing issues that cannot be corrected

However, the FAA allows for Special Issuance medical certificates in some cases. If you have a condition but are otherwise stable and functioning, a HIMS evaluation may help you obtain clearance.
